You must have all required documents ready to upload before starting the application.
The application typically takes up to 30 minutes to complete.
Each section of the application requires at least one supporting document to verify eligibility and confirm that program guidelines are met.
Applications cannot be reviewed until all required documentation is received.
Required Documents
- Proof of Military Status (one of the following)
- DD-214 showing Honorable Discharge, or
- Active-duty orders or valid military ID, or
- Other official documentation confirming military service or dependent eligibility
- Proof of Florida Residency (Document must show a current Florida address.)
- Florida driver’s license, or
- Florida State ID
- Proof of Household Income (Most recent documentation.)
- Pay stubs, or
- VA benefits letter, Social Security award letter, or unemployment documentation, or
- Other official documentation verifying current household income
- Proof of Hardship (Document must show what occurred and when.)
- Documentation related to the hardship, such as a medical bill, repair estimate, notice of unexpected expense, court documents, funeral arrangements, death certificate, or similar records
- Proof of the Expense for Which Assistance Is Requested
- Current bill, statement, or ledger showing the amount due, due date, and payee information, and
- If applicable, a disconnect notice, late notice, or past-due statement
